Upper Arm 
The time to deal with fat or cellulite in the upper arms is before the skin becomes loose and wrinkled with age. Fat above the elbow can be removed by liposuction, using one tiny, quarter-inch incision that will disappear without leaving a scar. If you address the problem while you are young and the skin is still elastic, the skin will shrink spontaneously and the problem will be solved. As long as you don’t gain a lot of weight, or gain and lose weight quickly with yo-yo dieting, the result of the arm reduction will be permanent.

However, if you wait until the skin on the upper arms is loose and flabby, then removing the extra skin will require an incision in the armpit several inches long. There may be a visible scar.

Bra Fat 
Bra fat” refers to the rolls of fatty tissue in front of the armpit that are very obvious in strapless and sleeveless clothing. This fat can be removed permanently with a tiny hidden incision and liposuction. The procedure is performed under a local anesthetic, and you can resume normal activities the next day without discomfort. The final result manifests in one to two weeks.


Back Fat
The upper back and underarm areas can be problematic when excess fat deposits bulge over clothing and bathing attire. These bulges have many names, but are all “blocked fat” areas that can be difficult to manage with normal weight loss and exercise programs.Topical cellulite treatments like Endermologie can be of help when the excess fat is not too extensive. However, even women of normal weight can have significant fat deposits.

Liposculpture — liposuction using a very small cannula, or tube — is a very safe and effective long-term solution to persistent problems in these areas. The incisions are small — less than an eighth of an inch — and can be hidden in the skin folds. The procedure is done on an outpatient basis, with sedation. Post-operatively, a simple compression garment, such as a body stocking, is worn for five to seven days. Pain and bruising are minimal, and most people can resume normal activities in three days. Final skin tightening takes eight to ten weeks.

The best candidates for this procedure are people with a moderate amount of fat and good skin tone. They can expect to see excellent skin shrinkage.

A breast lift or reduction is a procedure you can consider no matter what your size. This operation has much in common with smaller breast reductions, but normally no tissue is removed, and incisions are much smaller and easier to conceal. Occasionally, a small breast implant is added to fill out the upper portion of the previously sagging breast.

 

Even small breasts can sag, and when the nipple starts to migrate down toward the fold under the breast, it usually means that more than an implant is needed to fill in the discrepancy between skin and breast tissue. A careful surgeon will use an implant only when the problem is not severe. If too big an implant is used, an unnatural result will occur. Careful planning by the surgeon and patient is necessary to ensure the desired results.

In minor sagging cases, an incision is made around the nipple, and the extra skin is tucked in toward the nipple. This procedure can do an excellent job of restoring breast contour by bringing the skin and breast tissue into youthful balance, but it cannot beused with extensive drooping, or a too-flat breast will be the result. The scar around the nipple be visible only when the breast is completely exposed.

In more severe sagging, it is necessary to extend the incision from the nipple to the under-breast fold to deal with the extra skin. A vertical incision from the nipple to the fold removes and tucks breast tissue and skin. The scar will heal very well and be inconspicuous. Internal breast sutures do an excellent job of creating a youthful, self-supporting breast shape, so there is no tension on the skin surface to cause the scar to widen.

Even in more severe cases, a small horizontal extension of the incision into the fold under the breast will not be visible in most types of bathing attire. The freedom from fill-in foam or push-up bras is well worth the effort. The secret in correcting more advanced cases of drooping is the sophisticated suturing inside the breast tissue, which replaces the support lost when collagen bands stretch out with age.

Don’t be averse to a small saline implant under a lift, for it will not be visible and is almost impossible to feel. It will maintain cleavage even in bra-less situations.

 

PRE-SURGERY
Patients with history of cysts may be required to have a mammography. Patients should not be obese. No anti-clotting agents should be taken.

 

POST-SURGERY
Use a post-surgical garment (support bra) for five days, then no underwire bra for six to eight weeks. Avoid strenuous arm lifting or sports, like tennis or golf for four weeks. Always wear some type of support garment when active. Sutures will dissolve.

Decolletage.


Scarves and turtlenecks are one way to hide signs of aging on your neck, but it is possible to unwrap yourself. The upper chest and lower neck (décolletage), in women, often show the most dramatic premature aging, in fair-skinned individuals. A combination of factors are usually involved: loss of collagen and elasticity leading to lines and wrinkles; development of brown/age spots, broken blood vessels (Telanglectasias), and rough spots (Keratoses) leading to uneven texture and pigmentation; and changes in the underlying muscles leading to bands in the neck.

 

Since so many changes are occurring, multiple treatment modalities are needed to completely rejuvenate this area. Luckily, there are many options that can be done with little or no downtime.

 

The loss of collagen and elasticity can be addressed with Fractional laser or IPL (Intense Pulsed Light), topical preparations, and chemical peels.

Brown spots, Telangiectasis, and Keratosis can be treated with IPL, with or without the addition of Levulan (Aminolevulinic Acid) and/or a combination of. Vertical lines in the neck (Platysmal Banding) can be addressed with >Botox injections, or, if more severe, surgical tightening.

The décolletage is an area that is often ignored in the rejuvenation process, but it can be treated easily and should be a part of any rejuvenation program.


The cup ear, or lop ear, deformity is congenital and can be hereditary. Although it is evident from birth, but it is best to do nothing until the child is about six years old and the ear is large enough to permit correction. Seldom is a child under six self-conscious enough to express concern about the appearance of his ears.

 

Various procedures are used fix these deformaties. In most cases, the result is a very natural and inconspicuous ear. Only in extreme cases – when the ears are miniscule or severely underdeveloped – is complex cartilage-graft structural reconstruction needed. The same operation can be done on adults in the case of prominent ears.

For most woman, common ear correction in women’s ears is the repair of split earlobes or elongated holes that have stretched from wearing earrings. These can be fixed easily under straight local anesthesia with no loss of work time. The sutures are removed in five days, and the ears can be re-pierced in about six to eight weeks. After that, the studs are left in for four weeks to allow the holes to mature. After that, you can wear any earrings.

 

Too-long earlobes can be shortened with a simple but careful out-patient procedure done under local anesthesia. The results are permanent, youthful looking earlobes. Dressings are worn for about a week although swelling may persist for several weeks. The sutures dissolve.

Figure restoration has come to mean a lot more than just liposuction or a simple tummy tuck. While these procedures work well in minor figure problems, the lax abdomen and unsightly “spare tire” that can appear after childbirth, or a c-section, or other surgeries, require more sophisticated corrections. Figure restoration is a comprehensive approach to dealing with several areas of blocked fat that need shaping up.

 

When diet and exercise fail, new and safe surgical techniques allow for a permanent and complete return to a youthful or pre-pregnancy figure. The problems to address are usually three-fold: permanent stretching of muscles, loss of skin elasticity, and “love handles” or a “spare tire”.

 

The more advanced lateral tension abdominoplasty allows for extensive lifting of the thighs and buttocks, and total removal of love handles and the folds above them. A true waistline can be restored and the muscles tightened. In this procedure, the abdomen can be sculpted with liposuction to give definition to the muscles.

 

Figure Restoration is a broad category which encompasses extensive use of liposuction and skin excision. The skin excision can vary from a simple tummy tuck to an extensive figure restoration, including lifting of the thighs and buttocks. These procedures are usually reserved for women who have completed their childbearing, but in isolated cases they can be done even in women contemplating further pregnancies who have significant problems in the midriff.

PRE-SURGERY
In the traditional tummy tuck operations, patients need to be within 20 percent of the ideal weight for their height, but the more advanced lateral lift operation with liposuction can be done safely and effectively on patients who are thirty or more pounds overweight. The patient must be in good health and carefully selected. Evaluation for abdominal hernias is a must. Patients must stop smoking and discontinue blood pressure medication.

 

POST-SURGERY
One can expect ten days to two weeks of recovery before returning to normal activities. Athletics may be resumed after four to six weeks. However, the need for aggressive torso exercises, is basically eliminated because the abdomen has to be returned to pre-pregnancy conditions. Carbon Dioxide (CO2) Laser
The carbon dioxide (CO2) laser has been the premier skin resurfacing tool for the past decade. Its primary advantage over peeling and other laser techniques is that it tightens the skin naturally, while stimulating much collagen production. The results approximate that of a mini-facelift, without incisions.

 

The rejuvenating effects of the CO2 laser are aesthetically pleasing and will last for many years. In the hands of a skilled practitioner, the technique is very safe. The procedure is done under a local anesthetic, with sedation by an anesthesiologist, and takes less than an hour to complete. New methods of post-laser care have significantly shortened the period of redness following a CO2 laser treatment. With make-up, it is possible to return to work in about a week.

 

For correcting mild wrinkles, the patient has several options and should carefully consider the risks and benefits of each. But when the wrinkling is more advanced and the skin begins to lose elasticity, then the CO2 laser, with its longer healing time, is a far better choice. PRE-SURGERY
The patient must have a proper cleansing routine appropriate for their skin. All steroid use must stop. Retin-A use should stop two weeks before treatment, and Accutane use should stop six months before treatment. The patient should avoid exposure to the sun. CO2 laser treatment is best done without a suntan.

 

POST-SURGERY
The patient must take meticulous care of dressings. Redness may persist for longer periods, but can be camouflaged with proper makeup. The patient should stay out of the sun for five to seven days after treatment.

 


Profect Laser
Fast and painless, the Profect laser is the first selective non-ablative laser approved by the FDA for wrinkle reduction and acne treatment. “Non-ablative” means that unlike most lasers, the Profect triggers a healing response in the skin without damaging the superficial layer. In addition to wrinkles and acne, the Profect laser targets fine lines, rosacea, facial capillaries, and acne scarring. When combined with microdermabrasion and Levulonic acid, the Profect laser is highly effective at correcting even the most bothersome skin conditions. PRE-TREATMENT

There are some areas of the body that diet and exercise cannot fix, but that doesn’t mean you can’t have shapely thighs. “Saddlebags” — those unsightly bulges on the outer thighs — are mainly a female issue. And they’re as troublesome as love handles for the same reason: fat in these areas is “blocked” fat — six times harder to lose than fat in other areas of the body, and virtually diet-resistant. However, if a woman has good skin tone, once the fat is removed by liposuction, the skin in the thighs will contract to form a pleasing contour. In most cases the results are permanent.

Bunions

What is a bunion?
A bunion is an abnormal enlargement of a bone, usually the metatarsophalangeal joint at the base of the big toe. Bunion is the Latin word for “enlargement.” If the base of your big toe protrudes from the side of your foot, and the big toe is angled toward the other toes, you most likely have a bunion.

 

 

A bunion is formed when the normal balance of the foot is offset. The big toe joint carries a majority of the body’s weight when we walk and run. If the foot is unstable in some way, the big toe may begin to deviate–to separate from the other toes at the joint. A bony bump develops at the base of a big toe.

 

 

When a bunion forms on the little toe, it is called a “Tailor’s bunion.” It was given that name, it is thought, because tailors used to sit cross-legged on the floor, which sometimes led to the formation of a bunion on the outside of the foot.

 

 

What causes a bunion?
There are several factors that can cause bunions. The first is heredity. Just like eye color and hair color, the structure of your foot is predetermined. Family members who share the same faulty foot anatomy may also share a tendency to develop bunions.

 

 

What role do high heels play in the formation of a bunion?
Heredity alone does not cause bunions, however. Some develop, or are exacerbated by, wearing improperly fitting shoes. Unfortunately, stylish shoes are seldom designed to fit the normal foot. High heels throw the weight forward, increasing pressure on the big toe. Shoes with a narrow, pointed toe box compress the toes. If the foot is structurally predisposed to problems, the constriction may cause a bunion to develop. People with flat feet or pronated ankles are more likely to develop bunions.

 

 

An active lifestyle may lead to bunions in some people. Ballet dancers, particularly those who dance en pointe (in toe shoes), often develop bunions.

 

 

Is a bunion painful?
Bunions can be uncomfortable, either because there is pain in the joint itself or because the misalignment of the foot leads to pressure and rubbing from shoes. Corns and calluses may develop on the big toe or on the bunion itself, or on other toes that have been displaced by the bunion. How much pain is experienced is a factor of how severe the deformity is and what sort of shoes are worn. If the bunion immobilizes the joint of the big toe, arthritis may form in the joint, causing pain.

 

 

How do I tell if a foot problem is a bunion?
Symptoms include:
• A bony protrusion on the big toe that is uncomfortable while wearing shoes
• Red, callused skin along the base of the big toe
• Pain over the joint of the big toe (or less frequently, the little toe), aggravated by pressure from shoes
• Big toe turned toward the other toes

 

 

Tests
A physician will usually make the diagnosis of a bunion following an examination of the foot and an x-ray to note any abnormal angle between the big toe and the rest of the foot.

 

 

Treatments
Conservative treatment of bunions includes:
• Properly fitted shoes, usually with a low heel and an ample toe box
• Felt or foam pads to protect the bunion
• Splints to straighten the big toe and separate it from the second toe. (These are worn at night and may be somewhat effective for young people whose bones are not fully formed, less so for adults.)

 

 

Is bunion surgery cosmetic?
If conservative therapy fails, surgical intervention may be required. Only your physician can make that determination.

 

 

Surgery is the only way to completely remove the deformity. During the procedure, the bunion is removed and the bones of the big toe are realigned. Thanks to recent innovations, including absorbable screws and stitches, surgery can be performed under local anesthesia, with standby sedation. The patient walks in and out of the operating suite on the same day.

 

 

The severe pain previously associated with bunion surgery is a thing of the past. The use of intra-operative medication (medication given during the surgery) greatly diminishes the need for post-operative pain medication. Complete recovery in most cases takes between four and six weeks, but a quick return to normal activity is strongly encouraged. (A special boot protects the foot.) With the new technology used in this procedure, the bones knit faster, and weight can be put on the foot sooner.

 

 

Now, the goal of wearing a stylish shoe in comfort can be achieved with minimal discomfort and time away from normal daily activities.

Over the past twenty-five years, liposuction has come to mean the removal of fat deposits with a hollow cannula, or tube. Liposuction is a generic term that does not take into account the many improvements that have made the removal of fat through tiny incisions the most popular aesthetic procedure done today.

 

How do I know if liposuction is right for me?

 

Ideally, these procedures achieve their best results in people who are within 10 to 15 percent of the ideal weight for their height. This is because liposuction best addresses regions that contain blocked fat, such as “saddle bags” and “love handles.” These areas cannot be well controlled by diet and exercise, as the fat cells they contain are highly resistant to being broken down for energy and are therefore referred to as “blocked.” A person has to lose six pounds of normal fat in order to lose one pound of blocked fat, so often, no matter how hard they try, abnormal bulges remain prominent. When attempts at controlling fat by diet have failed, it is best to address problem areas with liposuction as soon as possible.The longer skin remains stretched out, the less able it will be to tighten spontaneously following liposuction.

 

The incisions of liposuction are usually less than a quarter of an inch long and disappear over time. Recent advances in liposuction technology, including the evolution of very thin cannulas and minimally invasive methods, allow the surgeon to sculpt and contour abnormal fat deposits without leaving telltale grooves. This technique is referred to as liposculpture . Tiny instruments permit close contouring of the skin immediately below its surface, so the surgeon can create an ideal figure.

Will the skin shrink after liposuction?
When liposuction is done as a stand-alone procedure, it relies on the elastic properties of the skin to contract and conform to the reduced volume. During a pre-operative examination of the patient while she’s standing, the surgeon can determine how much the skin is likely to tighten post-operatively and whether or not an additional skin-tightening procedure might be necessary.

 

It takes up to two to twelve weeks for the skin to shrink completely and the liposuctioned area to reach its final form. In younger women who are neither extremely overweight nor post-partum, the results are generally very good. The skin overlying the liposuctioned area can be recontoured to achieve a near-perfect shape. However, if the skin is very lax or the cellulite is severe, some surface irregularities may remain after liposuction. Non-invasive treatments likeEndermologie , along with diet and exercise, can help deal with the problem. Older patients or others with significant extra skin might need an additional skin-tightening procedure, or lift.This can be done at the same time the fat is suctioned.

What is Liposuction?

 

 

A sexy derrière is always desirable, especially in the young. A shapeless or square behind is often the result of fatty deposits in three areas: above the buttocks, in the flanks or “love handle” area; below the buttocks (the so-called “banana roll”; and on the outside of the thighs (“saddlebags”).

 

Removing fat from these areas by liposuction while the skin is still firm can create a very pleasing profile without the need to make incisions in the buttocks. Lifting the buttocks themselves is normally not necessary, unless the person is of advanced age or has lost a great deal of weight. A buttocks lift requires significant incisions, so the procedure should not be considered lightly.

 

The love handle/saddlebag/banana roll regions shrink very rapidly after liposuction, often revealing a very shapely behind that has been hidden under fat deposits that would not respond to normal dieting.

 

If the derrière is sagging, fat injections can be added to increase volume. This can be done at the same time as the liposuction procedure.

 

These simple, outpatient operations are done under local anesthesia . Recovery time is rapid – five days on average – and the results are superb. If done before sagging is advanced, the results will last for many years.

Endermologie is performed in a series of up to sixteen weekly sessions, then maintained by diet modification and once-monthly sessions. Each session normally lasts approximately thirty-five minutes and can be combined with an hour-long topical non-injection mesotherapy session for state-of-the-art cellulite therapy.

 

 

While not a substitute for liposuction in treating larger fat deposits, pre- and post-liposuction Endermologie sessions can be very effective in enhancing the results.


Like other plastic surgery procedures, the old-fashioned tummy tuck has evolved. In its simplest form it can still correct small bulges in the lower abdomen and muscles stretched out by childbirth. But now, when combined with extensive liposuction and thigh-lifting, the tummy tuck can completely restore a figure that has lost its shape and tone after multiple deliveries, C-sections, or other surgeries.

 

Fat deposits that have settled in the hip and thigh areas (“love handles” and “saddlebags”) often combine with lower abdominal fat to create the proverbial “spare tire.” Traditional abdominoplasty incisions stop well short of correcting these problems, and liposuction can help only up to a point.

 

The newer, more advanced, lateral tension abdominoplasty allows for extensive lifting of the thighs and buttocks, as well as total removal of love handles and the folds of tissue above them. The procedure restores a true waistline and tightens the abdominal muscles. The abdomen can then be sculpted with liposuction to give definition to the muscles.

Recovery involves gradual mobilization over the two to three days following surgery. The patient must wait up to one month before returning to strenuous activities.

 

 

Mini TummyTuck
Mini tummy tucks only remove the skin below the belly button and under most circumstances do not surgically involve it. They involve shorter scars than full tummy tucks usually and heal much faster. Muscle tightening, when necessary, is restricted to the muscles of the lower abdomen.

 

Mini tummy tucks are normally combined with liposuction of the full abdomen for very effective body contouring, and work best when there is minimal skin excess above the belly button. The scar from the skin removal is discretely placed in the fold above the pubic area and hip bones; it can often be hidden under small bikini bathing suits or under garments. In a mini there is no belly button scar. Love handles can be removed with liposuction as the inner thighs all using the mini scar.

 

Many patients are good candidates for mini tummy tucks, a consultation with an experienced plastic surgeon will be the best way to determine if you are. If you are within 10 percent of your ideal weight with no distention of your upper abdomen, and don’t have multiple skin folds above your belly button you should inquire if this procedure works for you. Skilled plastic surgeons are doing more with less and the combination of mini tummy tuck and liposuction can have dramatic and permanent results.

 

These procedures are done as an outpatient usually in two hours or less, they require much less anesthesia than full tummy tucks, and are very safe. Most patients are active the next day recover at home, and return to work in about a week. A support garment (light weight) is worn for 1 to 2 weeks, after which exercise is allowed the results are visible very early.
